If you’ve never scheduled an appointment with us before, or it has been more than 3 years since coming in, you will need to call 503 282 5358 or email email@example.com to schedule that first appointment.
Please specify if you plan to use insurance or pay out of pocket. If you plan to use insurance, we will need to check your benefits before you come in. In order to check your insurance benefits we will need your name that is currently used on your insurance plan, your date of birth, and the member ID on your insurance card.
Once you have a confirmed first appointment through our front desk you will be sent a link to create a login through our OnPatient portal. This will create an encrypted connection for you to communicate with us, check your labs, and schedule or cancel future appointments and will be connected to your electronic health record with us.
You may then use the onpatient login button under Existing Patients for all of those functions.

Existing patients can access their information and schedule additional appointments by logging into OnPatient using the link below.
If you are looking for your lab results, login to OnPatient, click on “Lab Access,” and then click on your name.
If this is your first visit and you’ve received confirmation of your appointment (after scheduling under New Patients), please set up login information with the OnPatient link below, click on “Appointment,” and then click on “Check In” to access your new patient forms.
